Hyperkeratotic papular rash in a patient with chronic mucocutaneous candidiasis

A case is reported in a 27-yr-old man from Pennsylvania, USA, with the early onset form of chronic mucocutaneous candidosis who presented with a 2-month history of progressive odynophagia, weight loss, cough and a diffuse pruritic eruption. Examination revealed white Candida plaques in the oropharynx. Cutaneous examination revealed numerous excoriated erythematous 1-2 mm papules on the trunk and extremities. Thick, yellow-white scale was present on the penis, scrotum, medial thighs and in the web spaces of the hands and feet. The patient's odynophagia resolved after 10 d of treatment with amphotericin B followed by oral ketoconazole. Scrapings of the skin lesions revealed numerous ova of Sarcoptes scabiei and crusted (Norwegian) scabies was diagnosed. Skin lesions resolved after treatment with lindane solution applied topically at night and washed off 8-12 h later. This was repeated the next night and 1 wk later.
