The authors present a case of esophageal stenosis caused by Paracoccidióides brasiliensis. The patient was referred to our hospital with the diagnosis of cancer of the esophagus, but his good shape made us investigate other, possible causes. The value of esophagoscopy and repeated biopsies is discussed. A clinical report is presented, as well as a discussion on the diagnosis of South-American blastomycosis of the esophagus. The treatment was based on sulfa drugs, cortisone, anti-blasto vaccine. Endoscopic dilatation of the esophageal stenosis with Plummer-Jackson bougies was associated. The patient has improved regarding his disphagia, but is still in treatment.
