Two cases of familial metaphyseal dysplasia are described. This is an anomaly of the skeleton in which, during the growth, there is not the transformation of the metaphyseal bone in diaphysis. Therefore large segments of diaphysis maintain the structure of the metaphysis: they are composed by a spongy bone tissue, they have a thin cortical, they are without medullary channel. The anomaly is due to the lack of functional adaptation of the bone to the mechanical exigencies. The observation of the radiographic picture permits to identify in which measure each epiphysis contributes to the growth ill length of the bones.
