Perianal fistula and carcinoma together are rare. There have been only four previously reported cases with Mycobacterium tuberculosis as the etiologic agent of the fistula. Chronicity and previous or multiple fistulas may be findings which suggest a problem other than an uncomplicated fistula. When these are present, careful evaluation by extensive culture and biopsy is necessary to rule out malignancy or tuberculosis. Treatment consists of adequate drainage, antituberculous drugs, and definitive operation when the tuberculosis has been controlled.
