The Author reviews the historical points of the problem of the tuberculous ano-rectal fistulas. He tries to clear up the subject, removing the confusion, face to the bacillar etiology of certain fistulas. Afterwards, he reviews the statistical references of some of-. the most outstanding authors, showing the controversy of their conclusions. He Calks the attention to the several conceptions and ideas submitted to the evolution and advancement of this subject. In following, the A. studies the way how the Koch bacilli from the lungs come to the ano-recta] canal. Concerning its etiology, the A. reminds, based on the authority of Samuel Cant that the Koch bacilli are respOnsible .for the 2 % of the cases of fistulas. The A. makes an accurate study of the pathogeny, diagnosis and prognosis. He summarizes in a schematic picture all the characteristic signs which afford the differentiation of a tuberculous fistula from a common inflammatory one. In facing the treatment, he considers that this. problem belongs more properly to the preventive medicine than to surgery. The A. is of the opinion that the cure of the pulmonary tuberculosis, by clinical means, provokes the cure of the fistula. The A. criticizes the use of local anesthesia in operation for ano-rectal fistulas, whether are tuberculous or not.
