Effect of modified NLA with flunitrazepam and buprenorphine and surgery on adrenocortical function in man

Eighteen patients with a mean age of 50-years who underwent the abdominal surgery were the subjects of the study. They were premedicated with diazepam 10 mg and atropine 0.5 mg im. Nine patients received droperidol-fentanyl-N2O anesthesia and other nine patients had flunitrazepam-buprenorphine-N2O anesthesia. Two ml of blood was withdrawn on five occasions as follows; before the induction of anesthesia, 30 min after the induction, one and two hours after the start of surgery, and at the time of adequate recovery from anesthesia in the recovery room. Plasma cortisol was measured for evaluation of the adrenocortical function. Flunitrazepam-buprenorphine-N2O anesthesia alone for 30 min did not exert any significant effect on plasma cortisol levels, but plasma cortisol levels increased gradually during surgery, as in the patients with droperidol-fentanyl-N2O anesthesia. The data obtained in this study suggest that the modified NLA with flunitrazepam and buprenorphine could be used as the original NLA with droperidol and fentanyl.
