1alpha-Hydroxycholecalciferol in children with renal osteodystrophy

Of 10 children 5 to 18 years old with renal osteodystrophy 5 were treated by chronic haemodialysis, 1 by chronic peritoneal dialysis and 3 by diet alone. Previously 6 had been given ergocalciferol; 2 were on anticonvulsant treatment and all were given aluminium hydroxide to maintain serum inorganic phosphorus between 1.25 and 1.75 mmol/litre. In 4 of 5 given 1 alpha -hydroxycholecalciferol absorption of calcium increased. On the basis of decreases in serum alkaline phosphatase values from above 280 towards normal 140 IU/litre, there was evidence of healing in only 1 of 5 children treated for less than 6 months but all 5 treated for more than 6 months had a definite response to treatment. There was a significant increase in serum Ca after treatment for 1 month. In most children there was some radiological improvement after 3 months and all showed definite improvement after 6 months. In 5 of 8 patients serum immunoreactive parathyrin decreased.
