1alpha-Hydroxycholecalciferol in renal osteodystrophy

Studies on 30 patients, of whom 23 were on regular haemodialysis 3 times a week with a dialysate concentration of 1.55 mmol/litre and 7, 2 of whom had renal rickets, were managed conservatively, confirmed the beneficial effect of 1 alpha -hydroxycholecalciferol (1 alpha -HCC) in nondialysis renal osteodystrophy; clinically all 7 patients showed improvement in musculo-skeletal function, best in the 2 children with renal rickets. The osteomalacia of the haemodialysis patients did not improve on treatment with 1 alpha -HCC alone, but results suggested that the failure of 1 alpha -HCC to promote healing of osteomalacia in those patients was not because of lack of action by 1 alpha -HCC, but that the osteomalacia was largely the result of depletion of phosphate.
