Novobiocin in pediatrics, pharmacological studies: absorption, distribution, and excretion

1. Following oral Novobiocina doses we have studied its titers in serum, spinal liquid and urine from 32 boys between 2 to 11 years old. 2. With single doses of 3, 6, 12.5 and 25 mg/kg we would prove the following: a) Novobiocina is rapidly absorbed, and within the first 60 minutes it gets serum active titers. b) High rate titers are obtained within 2 hours; 4.12 mg/ml with 3 mg/Kg; 5.05 mg/m1 with 6 mg/Kg; 43.15 mcg/ml with 12.5 mg/Kg, and 55.0 mg/m1 with 25 mg/Kg. c) Giving 3 and 6 mg/Kg doses, titers are extremely low after 6 hours; with 12.5 mg/ml in 12 hours are above 10 mg/ml. During the first 4 hours concentrations are very high with 25 mg/Kg doses; since this moment levels decrease lower than those obtained with 12.5 mg/Kg suggesting that there is some related element which limits intestinal absorption. 3. Supplying 12.5 mg/Kg maximum rate serum titer it is 8 times higher than needed to inhibit the most gram-positive organisms, and 4 times, higher than required by Streptococcus faecalis, and the gram-negatives which are sensitive to its action. After 12 hours the titer exceeds its minimal inhibitory. 4. We found a certain cumulative effect after the fifth doses with multiple ones of 3 and 5 mg/Kg given every 6 hours. 5. Spinal liquid is not entered by Novobiocina. 6. Urinary titers are lower than those seen with cerum. 7. Accordingly to presented reports a useful therapeutic plan in the treatment of children infections would be as follows: 12.5 mg/Kg every 12 hours (25 mg/Kg/day) that theoretically would be better to 6 mg/Kg doses every 6 hours (nearly 25 mg/Kg/day). 8. It is advisable to supply other antibiotics agents and chemicals together with Novobiocina in meningitis and urinary infections,,,as it does not cross the meningal barrier and its urine titer is low.
