Clinical trials have shown that the newest 5HT3 antagonist, granisetron, is an effective antiemetic for both cisplatin-based chemotherapy and moderately emetogenic chemotherapy regimens including carboplatin and cyclophosphamide. Clinical trials have established that efficacy is dose related, with decreased efficacy at a dose of 2 or 5 µg/kg compared with a dose of 10 µg/kg. Similar response rates were seen when patients were treated with granisetron doses between 10 and 40 µg/kg. Granisetron is effective as a single-dose regimen administered over only 5 minutes compared with multiple-dose regimens or prolonged infusion times commonly used with some other 5-HT3 antagonists. Patient preference data show excellent patient acceptance of granisetron. Overall, these studies suggest that granisetron will be a useful addition to the oncologist's therapeutic armamentarium for the prevention of chemotherapyinduced emesis.
