Failure of topical amphotericin B in cryptococcosis

The case is described of a 45-y-old man, successfully treated for Hodgkin's disease, who, 7 yr later, developed an inguinal ulcer which showed typical Cryptococcus neoformans in biopsy and cultures. Cultural and immunological investigations for cryptococcosis were negative. The ulcer was resected after topical therapy with amphotericin B and did not recur. However, the patient died, aged 53, of pulmonary adenocarcinoma. Autopsy showed no trace of lymphoma or fungal infection by histology or culture. Histological examination of the ulcer, performed during the topical treatment, revealed that the yeast had survived intact throughout treatment.
