Acute regional ileitis due to anisakis larvae

A review of 1,531 cases of regional ileitis in Japan showed that most had been mistakenly diagnosed as Crohn's disease. The cases were divided into acute and chronic, and subdivided into 4 and 2 types, respectively. The acute cases had been classified as acute phlegmonous regional enteritis (107 cases), regional enteritis caused by a foreign body (140), acute exudative regional enteritis (622) and acute exudative regional enteritis with secondary bacterial infection (7). Chronic cases had been subdivided into Crohn's disease (107), inflammatory granuloma with eosinophilic leucocyte infiltration (120), and unknown (428). 196 cases of gastric and 77 of intestinal anisakiasis were also analysed and their distribution correlated with the distribution of infected fish. The clinical symptoms and the gross and histological findings of the intestinal cases are also presented. Experimental infection of rabbits suggested that intestinal anisakiasis may be an Arthus-type allergic reaction caused by reinfection with Anisakis larvae.
