The case is reported from Spain of a man who presented with abdominal pain in the lower right quadrant. An emergency appendectomy revealed erythematous lesions with accompanying adenopathy, and a 3-cm long worm was present in the most inflamed lesion. This was identified as a larval Anisakis simplex. The patient subsequently admitted eating [raw?] anchovies in vinegar. Larval extraction by endoscopy is the preferred treatment for human anisakiasis.
