Post-operative acute muco-erosive jejuno-ileitis with protein-losing enteropathy: recovery after partial resection of the small intestine

A 30-yr-old man with saturnism developed severe mucosal abrasion of the small intestine of unknown origin. He presented with acute dysentery accompanied by endotoxin shock, fecal loss of water and electrolytes and severe protein-losing enteropathy. Segmentary ileal resection, the limits of which were fixed by operative endoscopy, was performed. Recovery was complete apart from a well-supported short small intestine syndrome. The pathological findings were very similar to those of a unique observation of fatal mucoerosive jejunoileitis previously published. Nosologically this case may be compared with postoperative pseudomembranous enterocolitis, but necrotizing enterocolitis and nongangrenous ischemic enteritis must also be considered in the differential diagnosis.
