Changes of erythrocyte fragility and plasma hemoglobin concentration during administration of high dose fentanyl were investigated in 16 cardiac surgical patients. The fentanyl solution contains 100 .mu.g of fentanyl in 2 ml of distilled water. The patient was given fentanyl, 100 .mu.g/kg by means of an intravenous drip infusion over a 15 minute period. Blood samples were obtained before and after the administration of 25, 50, 75 and 100 .mu.g/kg of fentanyl. Erythrocyte fragility was measured by the Coil Planet Centrifuge method and plasma hemoglobin concentration measured by the ortho-tridine method. Increases of erythrocyte fragility were first noted after the administration of 25 .mu.g/kg of fentanyl, and the changes became remarkable with the administration of 75 .mu.g/kg of fentanyl. Plasma hemoglobin concentration was increased dose-dependently by the administration of high dose fentanyl. This study clearly shows that the rapid administration of high dose fentanyl causes low osmotic hemolysis in cardiac surgical patients.
