Cerebral circulation and metabolism during high-dose buprenorphine and fentanyl anesthesia

We compared the effects of buprenorphine 100 .mu.g/kg (B group) with those of fentanyl 100 .mu.g/kg (F group) on cerebral circulation and cerebral metabolism in mongrel dogs. Changes in CBF, CMBRO2 and CMRglucose were less in the B group and the time of maximal effect on the cerebral circulation and metabolism was between 90 and 120 minutes after the administration of the drug, whereas in the F group it was between 30 and 45 minutes. The decreases in CBF and metabolism were more related in the F group. PVR and SVR showed no changes in either group. The effect on the systemic circulation was less in the B group. Seizure activity was not detected in either group by conventional EEG or by Berg-Fourier analysis. The results suggest that high-dose fentanyl anesthesia may be superior to high-dose buprenorphine anesthesia as far as the cerebral circulation and metabolism are concerned. The time of maximal effect was found to be different in the two groups. On the other hand, when used with nitrous oxide, the systemic circulation seemed to be more stable during high-dose buprenorphine anesthesia.
