Convulsions after miconazole overdose

A case is reported in a 4-month-old Aboriginal male infant who received inadvertently 50 ml (500 mg) miconazole instead of the prescribed dose of 50 mg by intravenous infusion for suspected systemic candidosis. The infant experienced a 15-min generalized tonic-clonic convulsion, which began 10-15 min after the completion of the 1-h miconazole infusion. Miconazole therapy was re-instituted 36 h later without further incident. It is concluded that the miconazole injection is implicated as the causal agent of the convulsion, although it is not possible to exclude one or more of the other constituents of the injection fluid as a cause of the seizure activity.
