Renal responsiveness to magnesium and parathormone in primary familial hypomagnesaemia

In order to estimate the renal responsiveness to PTH in hypomagnesaemia, two girls with selective defect of magnesium intestinal absorption were loaded with magnesium and parathormone. There was no cAMP-phosphaturic response to magnesium-released endogenous PTH in the girl with decreased blood cell magnesium level and normal response to exogenous PTH in the girl with the normal one despite similar degree of hypomagnesaemia. These findings indicate renal unresponsiveness to PTH to be not a common feature in chronic hypomagnesaemia. Short immediate increase in urinary calcium excretion was observed in both girls after magnesium infusion independently of the pretreatment with PTH and the serum calcium level. The effect may be related to magnesium-dependent renal haemodynamic changes.
