Different immunologic methods were used to detect, identify and assay serum, urine and intracytoplasmic Ig and their L chains in 360 patients with varying paraproteinemic hemoblastoses (multiple myeloma, Waldenstrome's disease) and secondary and essential benign gammapathies. Immunoelectrophoresis and radial immunodiffusion with monospecific antisera against H and L chains of the main 3 Ig classes are the most valuable diagnostic tests; peroxidation of intracytoplasmic Ig verifies monoclonal proliferation of plasma cells. The importance of detection and identification of free L chains (Bence-Jones protein) in the serum is stressed.
