Ulcerating enteritis associated with flucytosine therapy

A case is reported in a 66-yr-old man with disseminated Cryptococcus neoformans infection who received therapy with oral flucytosine [5-fluorocytosine] (150 mg/kg daily) and intravenous amphotericin B (15 mg daily). The small bowel X-ray demonstrated severe luminal narrowing, ulceration and marked separation of loops of bowel. Ileal biopsy revealed an erosive enteritis with mucosal inflammatory infiltrate and disruption of the surface epithelium. Discontinuation of 5-fluorocytosine resulted in total resolution of the clinical and radiological abnormalities.
