Candida albicans arthritis treated with flucytosine

A woman with Bright's disease underwent renal transplantation. Three months later C. albicans was isolated from urine and synovial fluid of the right elbow joint. Intravenous amphotericin B was administered and the patient improved. Six months later inflammation of the right knee and elbow recurred and C. albicans was cultured from the knee. Amphotericin B therapy was reinstituted, but because the arthritis failed to improve it was discontinued and oral flucytosine [5-fluorocytosine] was started. The patient improved and after a further course of 5-fluorocytosine and synovectomy she recovered completely.
