The clinical course and autopsy findings of a case in which anemia and cavum mediastinale tumor were suspected clinically and autopsy revealed tuberculosis of the esophagus with an esophagus-trachea fistula are discussed. The case involved a 75 yr old man whose main symptoms were systematic weariness and coughing and on whom sufficient examinations could not be made because of the seriousness of the condition, although X-rays indicated a cavum mediastinale tumor. Autopsy results included miliary tuberculosis, extensive ulcer and esophagus tuberculosis which accompanied esophagus-trachea-hilum lymph node fistula formation.
