Novobiocin: a study of plasma and spinal fluid concentrations in man

1. Forty-one individuals received either single or multiple doses of novobiocin without any evidence of untoward side effects with the exception of vomiting in one individual. 2. Data are presented that indicate that an initial oral dose of 2 Gm. of novobiocin sodium produced promptly a high plasma level averaging 126.8 ug./ml. and subsequent doses of 500 mg. at 12 hourly intervals tended to maintain levels in the range of 20 μg./ml. or greater. Such levels are 4 or more fold those reported to be necessary for in vitro inhibition of the common gram-positive pathogens. The true significance of these plasma levels must be corroborated by clinical experience with the treatment of infections due to the organisms that are inhibited so readily in vitro. 3. In 24 individuals who had received single oral doses of novobiocin up to 2 Gin., no novobiocin was assayable in the spinal fluid despite plasma levels up to 90 μg./ml. The samples of cerebrospinal fluid were obtained within 2 to 7 hours after the administration of the dose.
