Clinical experiences with guaiacol-glycerine-ether as a muscle relaxant in 1575 eye operations

As shown by the course of 1575 different eye operations under the effect of guaiacol-glycerin-ether, the latter can be employed in ophthalmic surgery as a safe and non-toxic muscle relaxant by central reflex inhibition. Operations of all types of cataracts, glaucomas, retinal detachments and especially keratoplasties can be performed without akinesia or retrobulbar anesthesia if there is sufficient muscular relaxation and bulbar hypotony. The intraocular tension is diminished before cataract extractions in 79.3% by 2-4 mm Hg and before operations for glaucoma in 82.5% by 2-8 mm Hg. If statistics are compared the most common coniplications during the operation and particularly during the postoperative course are found to occur less frequently. In old cataract patients guaiacol-glycerin-ether endangers neither the circulatory nor the respiratory system and does not require stimulants, oxygen insuffiation or even intubation as with the use of curare drugs in ophthalmic surgery. An advantage of the above method is the more economical use of drugs for narcosis, such as Evipan, an account of mutual synergism. A further possibility of employment is nystagmus, if mh Operation is planned or the etiology to be eclucidated. As stagnation of nystagmus is achieved hereby without an effect an accomodation, experiments regarding the examination of objective visual acuity are worthy of recommendation. Disadvantages of guaiacol-glycerin-ether as muscle relaxant are: 1. The loss of time arising from the duration of injection which, however, can be compensated in large operating-rooms. 2. The more difficult grasping of the lens with the risk of rupture of the capsule due to bulbar hypotony. - On the "second eye" the experience already mode can be employed by advancing the start of the operation to the third to fourth minute of injection. The. two drugs MY 301 "forte" (Fa. Dr.Brunnengräber, Lübeck) and GGG "forte" (Chemische Fabrik VEB Berlin-Grünau) have the same and sufficient musclerelaxing effect in eye operations, if employed for premedication. While GGG "forte" influences the decrease of intraocular tension in a somewhat higher percentage of the cases, MY 301 "forte" is capable of exerting a more effectice hypnotic influence an the central nervous control of the turgor in the vitreous. Both drugs can be recommended as safe and equally valuable muscle relaxants for clinic and practice.
