The advent of an array of potent antiretroviral drugs, coupled with a deeper understanding of HIV- pathogenesis, has radically changed the approach to antiretroviral chemotherapy over the past 24 months. Recently, a group was impaneled by the National Institutes of Healths Office of AIDS Research to delineate an approach to antiretroviral chemotherapy based on the current concepts of the diseases pathogenesis (1,2). This panel developed a series of principles of therapy, summarized in Table 1 that provide a much more rigorous way of approaching antiretrovial chemotherapy. When applied to therapeutic decisions the principies can form an important basis for clinical decision making.
