Use of mitotane (o,p'-DDD) in adrenal cortex hyperfunction

The effects o f mitotane (o, p ' - DOD) therapy in 2 cases of adrenal cortex cancer and 3 cases of Cushing's disease have been described. The initial dose of the drug varied between 1.5 and 4.0 g per day, and the maximal dose between 4.0 and 6.0 g per day. Combined surgical and pharmacological treatment applied in two cases of cancer caused remissions lasting 1.5 years in one and 3 years in the other patient. In two cases of Cushing's disease a marked clinical improvement was noted. In one patient the reduction in blood cortisol level was accompanied by a retreat of advanced skin rnycosis. In another patient it was necessary to discontinue mitotane therapy because of the appearence of leukopenia. Lack of apetite was noted as a frequent side effect of mitotane.
