Respiratory effects of diviminol, a new analgesic

The respiratory effect of a new analgesic, diviminol, in single oral doses of 50 and 100 mg., the usual therapeutic doses, was evaluated in comparison with those of placebo and codeine 120 mg. in a double-blind crossover study carried out in 8 healthy volunteers. Before and at 2 hours and 4 hours after each of the 4 treatments the subjects' responses to a CO2 stimulus, provided by rebreathing in a closed circuit, were evaluated on the basis of the respiratory response curves (PACO2- VA) calculated from the data obtained. Displacement of the postdrug curves, change in slope and the area between pre- and postdrug curves were the 3 parameters used for assessment of respiratory effect. The effect of codeine on these 3 parameters was in the direction of respiratory depression and analysis of variance of the data at 2 hours regarding displacement, slope and areas between pre- and postdrug curves showed, in fact, that the behavior of codeine was significantly different from that of placebo and the 2 doses of diviminol. At 4 hours there were no significant differences in the effects of the 4 treatments. On the other hand, the two doses of diviminol and placebo had significantly different effects only on slope at 2 hours but not at 4 hours; after placebo mean slopes increased at 2 hours and 4 hours, while after diviminol they increased less and at 2 hours after diviminol 100 mg. the mean slope decreased slightly. On the basis of these results it is concluded that if diviminol has an effect on respiration it is clearly less than the respiratory depressant effect demonstrated in this study at 2 hours after codeine 120 mg.
