Widespread cutaneous cryptococcosis occurring in an immunocompromised patient treated with high doses of fluconazole for oro-pharyngeal candidosis

A case is reported in a 33-year-old HIV-positive man from Italy , who had been receiving oral fluconazole (200 mg/day) for oropharyngeal Candida infection, and who presented with severe weight loss, asthenia and fever. Intravenous fluconazole (400 mg/day) was administered but 2 days later the patient developed umbilicated papules in an eruptive pattern on his face, mouth and trunk. Cryptococcus neoformans was cultured from a specimen from a lesion. The intravenous fluconazole dose was increased (to 800 mg/day) for 1 week with no improvement. Symptoms resolved after treatment with intravenous amphotericin B (1 mg/kg daily) for 20 days.
