A three-week-old infant with invasive pulmonary aspergillosis

A case is reported in a 3-wk-old male infant from California, USA, who presented with cough and multiple large nodules on chest radiograph. Computed tomography of the chest demonstrated numerous bilateral large round masses and hilar lymphadenopathy. Cytological examination of bronchoalveolar lavage specimens demonstrated large amounts of hyphae. An open lung biopsy of a nodular lesion showed necrotizing pneumonitis and cultures grew Aspergillus fumigatus. Symptoms resolved after treatment with intravenous amphotericin B (total dose 267 mg) and flucytosine. An immunological evaluation revealed chronic granulomatous disease.
