Seventy-eight non-Hodgkin lymphoma (NHL) patients were treated immediately after clinical staging with an age-adjusted CHOP-schedule. Histologic subtypes according to the Kiel-classification were the following: centrocytic large cell type: 11 patients; centroblastic: 29 patients; immunoblastic: 18 patients; lymphoblastic: 9 patients; NHL high grade unclassified: 11 patients. Extranodal involvement was observed in 35 patients. Localized extranodal disease was seen in 12. Overall response rate was 85%. Relapses were seen in 12 of 42 complete responders after 2 - 19 months (mean time till relapse 7 months). Factors significantly influencing prognosis were response to the therapy (p<0.0001), Karnofsky status (p<0.0002), B-symptoms (p<0.001), LDH (p<0.002), clinical stage according to Ann Arbor criteria (p<0.004), blood sedimentation rate (p<0.004). Histologic subtypes according to the Kiel-classification, age and sex did not influence prognosis significantly. We did not observe increased toxicity in unselected elderly patients, which might be attributed to our dosage adjustment.
