The development of immunity to malaria is discussed from the clinical perspective. It is stated that one of the problems confronting immunologists and the development of a malaria vaccine is the lack of a good in vitro correlate of immunity. Premunition or asymptomatic infection with Plasmodium falciparum in childhood is achieved at the cost of considerable mortality. That mass vaccination with a partially effective vaccine leading to a reduction in malaria transmission could conceivably increase mortality is considered a possibility, and it is suggested that this problem be addressed before implementing vaccine programmes.
