5-Aminosalicylic acid and its metabolite in breast milk during lactation

Treatment in patients with ulcerative colitis or Crohn's disease with salazosulphapyridin or 5-aminosalicylic acid (5-ASA) is sometimes necessary in pregnancy and during lactation. A woman suffering from ulcerative colitis, who was taking 500 mg mesalazine (Claversal) 3 times per day during lactation, was observed. Time from last dose of mesalazine to sampling of breast milk and maternal blood was 5 h 15 min. The samples were immediately stored at -20 degrees C and later analysed by the HPLC method. In breast milk, only small amounts of 5-ASA could be detected, but concn. of acetyl-5-ASA were high, with a milk:plasma ratio of 5.1.
