A 60-year-old woman with a 25-year history of primary Sjögren's syndrome developed early sarcoidosis, with asymptomatic hilar and mediastinal lymphadenopathies and non-caseating granulomas; there was spontaneous remission of the lymphadenopathies in one year. The occurrence of sarcoidosis complicating long-standing primary Sjögren's syndrome has not been previously reported. A possible common immunologic abnormality is postulated.
