Intracardiac repair of congenital cardiac anomaly with atrioventricular and ventriculoarterial discordance

From 1970 to October 1986, 10 patients (male:6, female:4) with congenital cardiac anomaly with atrioventricular discordance (AVD) underwent intracardiac repair. The median age at operation was 16 years (range 5-42 years) and four patients had previously undergone Blalock-Taussig shunt in three and pulmonary arterial banding in one. One patient with double-outlet right ventricle (DORV) and pulmomary stenosis (PS) (IDD) underwent the closure of the VSD and pulmonary valvotomy through the anatomical left ventricular and had no complete heart block (CHB), but had tricuspid regurgitation (TR). In two patients with AVD and pulmonary hypertension (PH), and repair, of the septal defect was approached through right atrial (RA) incision and the sutures were placed around the left upper portion of the defect from the right ventricle and there was no CHB postoperatively. Six patients with AVD and PS (Single Ventricle [SV]:3, DORV:3) and one patient with SV and PH underwent modified Fontan procedures and had no CHB and TR. All of these ten patients survived the operation and much improved in clinical status after the surgery.
