The Authors report a Multiple Sclerosis case with acute onset, hemiplegia and aphasia, where a clinical and computed tomographic diagnosis of multiple cerebral metastasis had been initially made. All neuroradiological aspects of the disease are discussed, particularly referring to the problem of differential diagnosis of cases with stroke-like onset and to the workup which is followed to reach a correct diagnosis.
