A new case of gastric and peritoneal anisakiasis

A 45-year-old woman complained of acute epigastric pain 5 days duration. She had consumed raw fish 7 days earlier. Various examinations were inconclusive but fibroscopy revealed an area of severely inflamed gastric mucosa associated with a ball of parasites which were removed and identified as Anisakis sp. L4. Four months later, the patient developed pain in the right costal region and cholecystectomy was recommended. All pre-operative examinations, including fibroscopy, were normal. Exploration of the peritoneal cavity showed a mesenteric nodule which was removed; histology revealed heavy infiltration by polynuclear eosinophils, and epithelioid and giant cells. It is suggested that one of the gastric Anisakis larvae was not extracted and relocated in the mesentery.
