Successful repair of pulmonary sling associated with congenital tracheal stenosis

Eight-month-old boy was referred to us with a history of stridor since birth and frequent episodes of dyspnea. Bronchoscopy and bronchography demonstrated severe stenosis of the trachea. Pulmonary artery sling was noticed at the time of right lateral thoracotomy for the repair of tracheal stenosis. The reconstruction of pulmonary artery sling (the LPA-MPA anastomosis) and tracheoplasty with the costal cartilage graft were carried out simultaneously. Addition of external airway splint and aortopexy were required for residual tracheomalacia at 1 and 5 month following initial operation. The baby was finally extubated 170 days after the first operation and discharged home. As pulmonary artery sling carries high mortality and morbidity because of associated tracheobronchial anomalies, simultaneous repair of associated tracheobraonchial anomalies is essential in severe cases for the survival of the patient. To our knowledge, this is a first report in the literature of successful simultaneous repair of pulmonary artery sling and tracheal stenosis in infant.
