A successful complete repair of pulmonary atresia with nonconfluent pulmonary arteries in 6-year-old boy

Complete repair of pulmonary atresia with nonconfluent pulmonary arteries was successfully performed in a 6 yr old boy. He was cyanotic since birth and had undergone right Blalock-Taussig shunt operation at the age of 8 mo. On cardiac catheterization in July 1982, he had large VSD , pulmonary atresia and almost normal sized right and left pulmonary arteries at hilum that were nonconfluent. The source of blood supply to right pulmonary artery except shunt was from bronchial collateral arteries and left pulmonary artery from ductus arteriosus. Surgery was performed on Oct. 26, 1982 after interruption of a large bronchial collateral artery by embolization technique. Pulmonary arterial reconstruction was performed by a Y-shaped composite extracardiac conduit which was made of 20 mm Hancock valved conduit with side arm presutured to the periphery of the valve. The main conduit was passed beneath the aorta and sutured to right pulmonary artery and side arm to left pulmonary artery. The postoperative course was quite uneventful and he is tolerating ordinary school activity well at present. This evidently is the 1st successful complete repair of this type of anomaly in Japan.
