Twenty three upper abdominal surgical patients were studied to observe the suppressive effects of i.v. administration of diltiazem (a Ca blocker) on hypophyseal hormonal responses to surgical stress under neuroleptic anesthesia. Diltiazem was continuously infused at the rate of 30 mg/h for 2 h starting at the skin incision. Blood pressure and cardiac rate were successfully controlled by the i.v. infusion of diltiazem. Although plasma ACTH and ADH showed more significant elevation in this group earlier during the surgery. Diltiazem may affect the hypophyseal hormonal responses to surgical stress.
