Cryptococcal intracerebral mass lesions: CT-pathologic considerations

Two cases are reported. Patient 1, a 31-yr-old woman, complained of headaches of 6 weeks duration. CT scan revealed bilateral multiple round hypodense lesions with no enhancement. Cerebrospinal fluid cultures grew Cryptococcus neoformans. Despite treatment with amphotericin B and 5-fluorocytosine the patient died. Patient 2, a 42-yr-old man receiving immunosuppressive therapy, developed fever. CT showed a hypodense nonenhancing left thalmic and superior cerebellar lesion. Cryptococcus was isolated from suboccipital craniectomy specimens. The patient died. At autopsy the lesions in both cases proved to be gelatinous pseudocysts rather than granulomas or abscesses.
