Immune-mediated inhibition of autologous late erythroid colony formation (CFU-E) in 4 patients with pure red cell aplasia was studied. Inhibitory T cells were examined by pre-treatment of bone marrow cells with anti-lymphocyte globulin and complement and by removal of T cells from bone marrow cells. Inhibitory T cells were detected in 2 patients by these methods before starting the therapy. Hematological remission was induced with cyclophosphamide in 3 patients. During remission, autologous bone marrow cells were co-cultured with cryopreserved T cells or IgG. Inhibitory activity of T cells was confirmed in 2 patients and that of IgG in 1 patient. The inhibitory T cells and IgG could not be found during remission. Immune-mediated inhibition of erythropoiesis may be pathogenetic for pure red cell aplasia in some patients.
