Diagnostic value of determine gas exchange parameters in the evaluation of circulatory disorders in acute myocardial infarct patients with no clinical signs of circulatory failure

Patients (81) with acute myocardial infarction without clinical signs of circulatory insufficiency were subjected to a many-parameter examination of gas exchange within the first 72 h of the disease to appraise the hemodynamic efficacy of different magnitudes of the minute heart volume. The cardiac index of < 3 l/min per m2 was recorded in 33% of the patients, that from 3-4.5 l/min per m2 in 38%, and that > 4.5 l/min per m2 in 28% of the patients. As the cardiac index increased, the total peripheral and pulmonary vascular resistance declined, the blood volume and blood flow rate in the lungs rose, while the parameters of ventilation and pulmonary gas exchange function, gas composition of the arterial blood, acid-base balance and lactate/pyruvate ratio changed equivalently.
