Fibrinolytic system of the blood in peptic ulcer complicated by hemorrhage (retrospective data)

The kinetics of fibrinolysis in 206 subjects (20 healthy subjects, 81 patients with duodenal ulcer with a history of hemorrhage and 105 patients with unaggravated peptic ulcer) were studied in regards to the ulcer site, gastric secretion and blood group. The patients with peptic ulcers showed a tendency towards activation of the fibrinolytic system (FS) which was more remarkable in patients with a history of hemorrhagic complications and basal hypersecretion, with the greatest shifts being disclosed in patients with peptic ulcers in the anastomotic area. The prognostic signs that attested to the possibility of the development of gastrointestinal hemorrhage in patients with unaggravated duodenal ulcer involved a reduction of the AB period (< 110 min) and .epsilon.-ACA hypersecretion. Unfavorable factors that contributed to recurrent hemorrhages in patients with duodenal ulcers involved a reduction of the AB period, the absence of hyperfibrinogenemia, positive familial anamnesis, I (0) blood group, basal secretion of acid and hyperplasia of the parietal cells of the gastric fundal glands. The effect of .epsilon.-ACA depends on the initial status of the FS.
