Fatal hepatitis during sulfasalazine treatment

Fatal hepatitis was seen with cytolysis, encephalopathy and hepatocellular failure in the course of a treatment with sulfasalazine (2 g/day for 3 wk) for Crohn's disease. The drug's responsibility seemed to be quite probable (hypereosinophilia, positive test of lymphoblastic transformation, positive investigation of circulating immune complexes). The histopathology was that of a necrosing hepatitis with inflammatory modification of porta hepatis. Comparison with 6 observations in the literature suggest an immuno-allergic mechanism.
