Glucocorticoid therapy of nephritis and the problem of drug resistance

A study of the number of 3H-dexamethasone binding sites in a cutaneous fibroblast culture of 14 nephritis patients with pronounced nephrotic syndrome showed that the number of binding sites was half that in patients who appeared to be resistant to long-term treatment with high doses of glucocorticoids. The same tendency was found in normal subjects. Evidently, some cases of resistance to glucocorticoid therapy are associated with a primary, genetically determined deficiency of cell receptors for the hormones, which can be used as an additional test in forecasting the therapeutic effects of high corticosteroid doses.
