State and effectiveness of dispensarization of patients with rheumatic diseases

A selective analysis of the condition and efficacy of prophylactic medical examination of rheumatic patients was made in 7 polyclinics of Moscow [USA] located in different districts over a 3 yr period. A total of 408 patients were examined, of these, 284 had rheumatic fever, 97 rheumatoid arthritis (RA) and 23 Bechterew's disease (BD). A long duration of prophylactic medical examination of rheumatic patients was noted. Most of the patients with RA and BD having a protracted course were taken under observation. The lowest index of temporary work incapacity was shown by patients with rheumatic fever. More than 1/2 of such patients had not taken sick-leave certificates for 3 yr. In RA and BD very high indices of temporary work incapacity were noted; this is attributed to late prophylactic medical examination of the patients, its short period (mainly up to 5 yr) and to an inadequately elaborated system of treatment at this level. The analysis showed the importance and necessity for further improvement of the system of prophylactic medical examination of patients with joint diseases.
