The mechanism of effect of hypoxia on pulmonary circulation is looked at in a comparison with theories on the Von Euler alveolar-vascular reflex. Clinical and experimental data from tests on humans, dogs and calves are cited to demonstrate which pulmonary vessels participate in the reaction to hypoxia. Partial pressure of O2, hypertrophy, pulmonary blood arterialization, ventilation, rate of change in respiratory phases and the exclusive role of alveolar hypertension in the pathogenesis of pulmonary hypertension are discussed. The development of pulmonary hypertension is the result of alveolar hypoventilation and is also pathogenically connected with all forms of systemic hypoxia. The effect of lactic acid and histamine on pulmonary pressure, angiotensin, prostaglandins, ATP formation and fat cells as a source of serotonin are also discussed. Data on the direct vasomotor effect on pulmonary vessel of a complex of metabolites, hormones and mediators, liberated in hypoxia primarily in peripheral tissues and organs, confirm the theory on pathogenesis of hypertension in pulmonary circulation. Pulmonary pathogenesis cannot be considered only on the basis of local mechanisms of respiration and circulatory interaction. Other mediators, hormonal reaction and hemodynamic interaction of pulmonary and systemic circulation must also be considered.
