The case is presented of a 30-year-old woman, from Venezuela, with Schistosoma mansoni infection and hepatosplenic syndrome, who developed a nephrotic syndrome with microhaematuria and moderate renal insufficiency over a period of about 4 years. Immunofluorescence revealed a deposit of IgG, IgM and complement in the peripheral capillaries and mesangia. Electron microscope studies showed the membranoproliferative glomerulonephritis, first revealed by light microscopy, to consist of an enlargement of the basal membrane due to the ingrowth of mesangial cells beneath the endothelium, which partially obliterated the capillary lumen.
