Surgery in cases of unusual forms of pulmonary aspergillosis

Of 124 operative specimens obtained by lung restriction from patients with aspergilloma, 7 single (1 cyst) and 3 multiple bronchiectasis specimens were selected. These showed obliterated bronchi filled with fluid from which Aspergillus was cultured. In the preoperative period no symptoms, many diagnostic problems, X-ray shadows similar to tumours and negative mycoserological reactions were observed. In only 4 patients serological reactions became positive after surgery. The types of surgery, postoperative course and follow-up results (1-12.5 yr) are described. In 9 patients the results were very favourable. Atypical aspergilloma is described as a tumour-like, blocked ectatic cavity filled with fluid infected with A. spores. Such a form, with mature mycelium, has not been described previously. It is suggested that A. spores deprived of oxygen are biologically different.
