Composition of Malaysian coconut water is given. A method of direct infusion from coconut to vein is described, with an account of its use in 15 surgical cases treated in an area without other facilities; it was clinically beneficial in 9 cases and had no effect on the other 6. Because of the high K content, 35.1 m-equiv/litre, it was used only when 24-h urine output was 1 litre or more. Pyrexia in 6 cases was transient. There was pain with infusion of 90 drops/min or over, but not with 40 to 60 drops/min. Commercial green coconuts aged 5 to 6 months were used, suspended by a loop of their own husk, punctured, then fitted with an infusion needle or large enough bore not to be clogged with particles from the coconut water.
